Next page required to fill the personal information (name, surname ).Next page you would have to fill the the name of relative of applicant and type of relation. This is to be mentioned both Malayalam and English .Next page you would have to fill the date of birth .Next page if you having any defect you should have to to fill the defect, your mobile number and email .Next page you should upload the supporting documents( photo, birth certificate Aadhar card). Select the birth certificate,and Aadhar card. you should select the village and state. Last section is a declaration of above information .submit the application.copy the reference number. you will track the status.
